The Lancaster Model

A digital health needs assessment for children and young people that identifies potential problems early on, so no child is left behind.

Digitally transforming healthcare

Developed over 24 years by Kath Lancaster and powered by Aire Innovate, The Lancaster Model (TLM) is an evidence-based digital solution designed to shift from reactive to preventive care. How does it work?

01

Children complete a digital health assessment that's processed in real time to provide immediate triggers and alerts.

02

Responses are scored and analysed through inbuilt evidence-based algorithms uncovering hidden needs.

03

Health professionals promptly deliver appropriate support and help before problems escalate.

04

Aggregated data creates anonymised population reports supporting collaboration and effective public health practices.

Transforming child safeguarding with Kent Community Health

Moving away from reactive safeguarding activities to proactive support of children and young people.

The Kent School Health team, part of Kent Community Health NHS Foundation Trust, works with children, adolescents and families with responsibility for approximately 300,000 children and 600 schools across the region.

Their role is to safeguard children and identify risks for early intervention and protection, including families, schools, the NHS and child protection services including the police when necessary.

Making children safer with the NHS in Derby.

We've created a solution that's improved the safety of over 14,000 children who have been assessed earlier enabling the appropriate intervention and preventative support, which may otherwise have been missed.

The NHS in Derby works with schools, social services and healthcare professionals to deliver School Nursing Support to Derby City’s 50,000 children annually between the ages of 5 to 19.

After being historically focused more on safeguarding they began a transformation during March 2020 to provide a more rounded, needs based support for the children and young people of Derby City.

Recognising that early intervention was of critical importance The Lancaster Model (TLM) offered a package of both service transformation and a digital solution to meet their needs.
